Abruptio placentae

Abruption of the placenta or abruptio placentae is a serious obstetric antenatal complication.
Separation of a portion of the placenta from the uterine wall, leading to bleeding. This may be revealed or concealed haemorrhage. A bleed near the circumference of the placenta will strip the edge off its attachment, decompress and reveal itself whereas a bleed starting near the centre may accumulate a large volume before the edge is reached, removing a large proportion of the area of the fetal life support system and causing great pain and a degree of shock in the mother which seems disproportionately greater than the volume involved.
Small abruptions are compatible with survival of the child, large ones have considerable potential to kill the mother as well.
History
Bleeding, or pain.
Examination
The uterus will generally be hard and tender in a concealed abruption, and the patient ill.
