Capgras syndrome (l’illusion des sosies) is a delusional syndrome (reduplicative paramnesia) where a patient assumes that a close friend or relative has been replaced by an impostor, or double, usually with no hallucinatory symptoms. It has occurred with most causes of delirium such as drug toxicity (eg lithium and ketamine), infections, folate deficiency, basilar migraines, myxeodema, strokes, metabolic encephalopathy, rarely after head trauma, as well as more commonly in functional and psychiatric disorders.
