Levels >10 mU/l

In the presence of normal or low free serum thyroxine are effectively diagnostic in adults of primary hypothyroidism and after elimination of reversible hypothyroidism will mandate lifelong replacement thyroid treatment [1].

In the presence of low free serum thyroxine are effectively diagnostic in adults of primary hypothyroidism and after elimination of reversible hypothyroidism will mandate lifelong replacement thyroid treatment[2].

Levels between 5-10 mU/L

With normal free serum thyroxine warrant checking thyroid peroxidase antibody. In the asymptomatic patient with no antibodies TFTs can be done 3 yearly, with antibodies yearly. The symptomatic patient can justify a 3 to 6 month trial of thyroxine, with good symptom response justifying lifelong replacement therapy[3].
