Introduction

Ibutilide is an antiarrhythmic agent that works by induction of slow inward sodium current, which prolongs the action potential and refractory period of myocardial cells. It is useful where initial electrical cardioversion has failed as in combination has a success rate in AF/A flutter of 100%. Magnesium supplementation makes the drug even more effective and safer.

Monitor for 4 hours after injection as 4% risk of torsade de pointes and a 45% risk of monomorphic ventricular tachycardia
