Loss of domain

In large abdominal herniae, displacement of abdominal contents into the hernia itself is termed loss of domain. This loss of domain occurs gradually as the hernia expands. Attempts to suddenly return this large volume of abdominal contents into its original compartment can cause respiratory and cardiovascular compromise. It can be regarded as a variant of abdominal compartment syndrome.

One approach to address this complication of hernia repair is to create progressively increasing pneumoperitoneum to re-create the space so as to accommodate the abdominal contents.
