The "statement of fees and allowances", which came as a red ring binder, with inserts that would be updated periodically, was the basis of the GP contract prior to the New Contract of {which year was it?}.

Although it is, in theory, out of date and defunct, it remains the only definitive source of guidance about certain legalisticall and sometimes complicated issues, such as which travel vaccines must be given free under the NHS, and which may be charged for.
