- Pregnancy - most common during labour
- HIV especially PCP (Pneumocystis carnii pneumonia)
- Pneumonia (particularily necrotising)
- Sarcoid
- Crytogenic pulmonary fibrosis
- Langerhan's-cell granulomatosis
- Lymphangioleiomyomatosis
- Tuberous sclerosis
- Rheumatoid arthritis
- Anklylosing spondylitis
- Polymyositis
- Dermatomyositis
- Scleroderma
- Marfan's syndrome
- Ehlers-Danlos syndrome
- Sarcoma
- Lung cancer
Incidence
Risk factors
- Smoking
- Tall - not as important as in primary spontaneous pneumothorax.
- Male 3.2:1 female
Management
Should be more aggressive than primary spontaneous pneumothorax as
- Poor pulmonary reserve
- High reoccurance rate
- Admit and observe for 24 hours after lung expanded- see Americian guidelines[4] and European guidelines[5]
- Observe if only very small and very stable patient
- Otherwise chest drain ...larger bore for larger pneumothoraces
- Consider (graded) talc pleurodesis on even first presentation
