Testicular torsion

Twisting of a testicle on its spermatic cord is an acute surgical emergency if the testis is to be saved. Even if the testis itself is found not to be viable at operation, it should be removed as the inflammatory reaction to the necrosed testis can lead to infertility in the remaining one.

Clinical features

  • Acute onset (with or without history of trauma) unilateral painful swelling of scrotum
  • Cord thickened and testis may be raised

Management

Exploration of the scrotum and orchidopexy.
