Zoster sine herpete

Zoster sine herpete is the classic dermatone pain without any diagnostic rash from varicella zoster. It is a difficult and often unsuspected differential diagnosis. It is only one cause of dermatomal pain but with the VZV reactivation there can be rising titres of VZV-specific antibody in both serum and CSF and patients can have VZV DNA in CSF and peripheral blood mononuclear cells detectable by PCR.
